If you elect an amount above the guarantee issue, you must complete an Evidence of Insurability (EOI) form if applying for this benefit outside initial eligibility as a new employee. It is your responsibility to complete the form and submit to HR.
 
While rates did not increase, as you age, your rates will increase when you enter a new age bracket.
 
Benefits available for spouse and children

If applying for Voluntary Life policy above the Guarantee Issue as a new employee or if you are a late entrant, it is the applicant’s responsibility to complete and return the Evidence of Insurability (EOI) form to HR within 30 days of application.  Benefits requiring EOI form are not confirmed until confirmation of policy placement received from Minnesota Life Questions?
