Add Beneficiary form and “Be sure to complete the beneficiary designation, as this would indicate who would receive the benefit in the event something happens to you.

If you request a Voluntary Life policy above the Guarantee Issue amount, an Evidence of Insurability form is required.  It is the employee’s responsibility to complete the EOI form and submit to Lincoln Financial.  Your policy is not in place until approved by Lincoln Financial.
