Dakota 911 offers four different HealthPartners High Deductible Health Plans (HDHPs) on the robust Open Access network.
Two of the plans are HSA-funded and also have a "buy-up" feature for certain preventative medications. The other two plans are HRA-funded.
Employees may elect to benefit from cost savings associated with participating in any of these plans through a smaller Select network of providers.
All plans require up front "deductibles" be covered by the employee at 100%.

Dental
Dakota 911 offers dental benefits through HealthPartners. The level of coverage depends in the provider you visit.
Before electing a plan, employees should research which benefit level their dentist is in.

Vision - Materials Only Plan
Dakota 911 offers a materials-only vision plan through eyeMed, part of the Mutual of Omaha suite. A materials-only plan helps to cover costs of lenses, frames, contacts and laser vision correction.

Life Insurance
Dakota 911 provides basic life insurance of 1x an employee's annual salary up to $50,000 at no cost to the employee. This coverage includes accidental death and dismemberment.
Employees may purchase optional life and AD&D for themselves. If elected, employee spouse & dependents also become eligible for optional life insurance.
Employee & Spouse optional life insurance rates are determined by the employees age as of January 1st of the plan year. Coverage levels are determined by the employees salary as of January 1st of the plan year.

Short-term and Long-term disability replace a portion of your income if you become disabled and unable to work.
Short-term disability is covered 100% by the employee.
Long-term disability is covered 100% by Dakota 911, but the premium is considered taxable income.

Dakota 911 provides long-term disability to all employees at no cost. The premium, however, is taxable so employees will see it listed on their check stubs for W2 reporting purposes.
