If preferred, the forms in the new hire binder may be completed & submitted electronically.
Any forms that need to be personalized will be included in the new hire binder or emailed directly.

Due First Week:

Dakota 911 is proud to offer eligible employees a comprehensive benefits package as briefly summarized in the annual enrollment guide. Employees share the costs of some benefits, and Dakota 911 provides other benefits at no cost. In addition, there are voluntary benefits (short-term disability, supplemental life and vision) with reasonable group rates that can be paid for through Dakota 911 payroll deductions. The 2024 Benefits Enrollment Guide recaps benefits options available to all full-time Dakota 911 employees. The following forms may also be required depending on your elections:

    • Evidence of Insurability Group #GVTL-ATL3 (only required if electing optional life insurance greater than the Guaranteed Issue)

2025 Open enrollment is November 1st - 10th, 2024. 

One-on-One Tele-meetings will again be offered during open enrollment, 2024. If you are interested in receiving a personalized review of 2025 benefit options, you will want to sign up for one of these appointments. Representative Sue Frick will be available for 20-minute phone or virtual meetings on the following dates/times:

The following information is everything you may need to support your 2025 open enrollment experience.
  
 
 
 
FORMS:
Benefit Election Form (emailed to you). - Required of all employees. 
 
Notices Acknowledgment - Required of all employees
 
     1. You wish to delay payout for use through the end of the year
     2. You wish to have your payout contributed to your Deferred Comp
     3. You wish to have your payout contributed to your Roth IRA
 
2025 Group Waiver Form - required if you are waiving medical coverage
 
VEBA Plan Change Form - required if you are moving from an HRA/VEBA to an HSA
 
     1. You are electing new life insurance of more than $5,000
     2. You are increasing current life insurance by more than $5,000
     3. You are increasing current life insurance, and you are over the guaranteed issue
  
Beneficiary Designation Form - only needed if you are changing your life insurance beneficiary information

  • Insurance card (or copy) must be provided
    • Acceptable Insurance: HealthPartners, Aetna, BCBS, CIGNA, Medica, Medicare (with supplemental), United Healthcare
  • Cost for anyone with other forms of insurance:
    • $40/Flu Shot
    • $45/FluMist
    • $70/Flu Shot Senior Dose (Ages 65+)
