Below is a copy of the "Equal to or Better Than" letter provided by Horizon which ensures our benefits will match the current SEHBP Medical and Prescription Plans.
How to Verify your Prescriptions with Horizon BCBS of NJ
Effective July 1, 2025, Fair Lawn Board of Education will be directly contracted with Horizon Blue Cross Blue Shield of New Jersey for Medical and Prescription benefits. Horizon utilizes Prime Therapeutics as their Pharmacy Benefit Manager (PBM). To verify coverage for your current medications, please follow the steps below on the Prime Therapeutics website.
1. Visit www.myprime.com/en/medicines.html#find-medicine
2. You will then need to select your drug list and select Apply.

3. Step 2 will prompt you to type in the name of your medication. You can also search for medicine based off the National Drug Code (NDC).
*Please be sure to select the right dosage when searching for your medication
4. Once the name and dosage of your medication has been selected, you will be able to see the Tier (Generic, Preferred Brand, or Non-Preferred Brand) and if the medication is on the formulary of your selected plan.
If you are unable to locate your preferred medication, please click on the Therapeutic Alternatives to find a list of similar drugs that will be covered under the plan. Should you still have an issue, please reach out the the Gallgaher Serice Team for additional assistance.

To locate a participating provider, visit www.Horizonblue.com and select FIND A DOCTOR at the top of the page.
On the next page, in the SEARCHING FOR EYE CARE PROFESSIONALS section of the page, under CHOOSE A PLAN select HORIZON FOCUS I and click on the CONTINUE TO DAVIS VISION button.
From here you can find a participating provider and access all of the added benefits that come along with your Horizon Vision Plan.

If you want to enroll or make a benefit change, you must complete the application located in Frontline.

The Frontline login can be found on the district’s website under “More”, “For Staff” and then “Frontline” or by clicking on the following link: Frontline - Sign In
Log into Frontline and make sure that your selection is “Frontline Central” using the drop down arrow.
Once in Frontline Central click on “My Forms” in the navigation bar on the left side of the screen.
Then click on the header “Forms I Can Start” listed in the center of the screen.
In the search bar type “Health Enrollment Form” to have the form populate for you to then start. Once the form populates, click on the “Start this Form” button on the right side of the screen.
Complete the form online with your medical/prescription selection and all required information and then scroll to the bottom of the form and click on the “Submit Form” button. The district will then process your application and contact you with any questions if necessary.

The file below will give you an estimate of the cost per paycheck for your medical options. You need to enter your estimated annual pensionable salary and select whether you are a 10- or 12-month employee.
All final payroll deductions are determined by payroll.
