Eligibility

 

Who May Enroll

  • Full-time or part-time associates working 30+ hrs./wk. – Eligible 1st of the month following 30 days for all benefits except disability. Associates become eligible for company paid disability after 1 year with the company. Associates must choose benefits within 30 days of hire date.
  • Eligible dependents – Includes Associate’s legal spouse and children to age 26, plus disabled dependent children of any age who meet plan criteria.

When You Can Enroll

  • New associates: Enroll within 30 days from your date of hire. If you don’t enroll within this time period, you will not have benefits coverage, except for plans and programs that are fully paid by HRI Properties, such as basic life insurance and short and long term disability.

Changes to Enrollment

  • After your enrollment opportunity ends, you will not be able to make changes to your benefits until the next Open Enrollment. The only exception is if you experience a qualifying life event, such as marriage, divorce, birth, adoption, or a change in your or your legal spouse’s employment status that affects your benefits eligibility. Documentation for all qualifying life events must be submitted to HR at your location within 30 days of the event. Life event coverage will begin on the first of the month after your ADP enrollment change is submitted and documentation has been approved, with the exception of the birth or adoption of a child which will begin on the date of this event.

Dependent Audit

After you enroll your dependent in either the Medical, Dental or Vision plans, you will have to provide documentation to verify them as your dependent. We work with BMI Audit Services to verify dependents to ensure your information remains confidential and secure. If your dependent is enrolled in any other coverage(s) other than Medical, Dental and/or Vision, you will need to provide dependent documentation to your Human Resources.

You will receive a personalized audit packet from BMI to your home within 1-4 months of adding a new dependent. The audit packet will include the following:

  • An audit letter that will explain the audit process and its requirements. The letter includes a toll-free customer service line for BMI Audit Services – please direct all requests for assistance to BMI.
  • A verification form that will ask you to identify your relationship to your enrolled dependent(s) and upload documents to BMI in order to prove the relationship. Examples of requested documents are: marriage certificate, birth certificate, etc.
  • A postage paid envelope (you will also be given instructions on how to fax or upload your audit submission if that is your preference)
  • If you do not have a required certificate or document copy, please order it immediately. The vital statistics website can help you determine the process for obtaining document copies. You may be required to contact the County Clerk’s office directly and there may be non-reimbursable costs associated with obtaining new copies.

Why Do We Verify Dependents?

  • Healthcare costs continue to rise for both employers and their employees. One factor contributing to the increase is the cost of covering dependents that are not eligible for plan benefits. As part of our ongoing efforts to control these costs and to keep our enrollment records up to date, we will perform an audit of dependents currently enrolled in our medical plans. BMI is an independent firm that specializes in employee benefits auditing services.

Already Submitted Your Documents?

  • Keep an eye out for a postcard that should arrive within 2 weeks after submitting your documents. If you don’t receive the postcard, please reach out to BMI.
