New hire enrollment must be completed within 30 days of eligibility. If you do not enroll in benefits within 30 days of eligibility, you will need to wait until the next open enrollment period to enroll in benefits.

Be sure to complete the beneficiary designation, as this would indicate who would receive the benefit in the event something happens to you.

If enrolling in Voluntary Life after initial eligibility please note that you may be required to complete an Evidence of Insurability.  This is the responsibility of the employee to complete and submit to carrier for approval.
