EXCITING CHANGES for 2025 Open Enrollment

Electronic Enrollment System

As you know, the district made an investment in an electronic enrollment system.  During open enrollment, you will enroll directly on the employee NAVIGATOR system.  This year open enrollment is ACTIVE / MANDATORY, which means, every eligible employee must log in to the portal and elect benefits effective January 1, 2025. IMPORTANT:  If you do not enroll in your benefits during open enrollment, you will not have ANY coverage for all benefits EXCEPT Basic Life/AD&D and cannot make changes for the remainder of the plan year unless you have a qualifying life event.   In Addition, IRS regulations require that you make an active FSA election for health care and/or dependent care each year.

If you are enrolling for coverage for the first time, the system will walk you through each benefit so that you can determine which benefits best meet your needs.  If you need additional assistance, an Educational Call Center will be available during open enrollment to answer benefit questions and how to enroll on the system.

Dependent Eligibility Audit will be conducted during open enrollment for those dependents NOT enrolled in the medical, dental or vision plans today:

  • To manage our healthcare costs, the district will be verifying the eligibility of dependent(s) newly enrolling in the medical, dental or vision plans. This will not apply if your dependent is currently enrolled in the medical, dental or vision plan.
  • You will be required to upload to employee NAVIGATOR the required documentation as listed below to validate your dependent’s eligibility for coverage under the medical, dental or vision plans.
  • Spouse
    • Marriage certificate listing date of marriage, or
    • Current federal tax return (front/signature pages only – black out financial information)
  • Dependent Children (whichever applies)
    • Birth certificate listing employee or spouse as parent
    • Adoption certificate/placement agreement
    • Documentation of legal custody/guardianship
    • National medical support notice/court order
  • Your benefits may be affected if there is a delay verifying the eligibility of your dependents. If you do not provide all the required documentation by the 12/6/24, he/she will not be eligible to enroll in the medical, dental and/or vision insurance coverage effective 1/1/25.

Information YOU MUST HAVE when enrolling your dependents

  • If you are enrolling your dependents for the first time, you will need to ADD the dependent name, relationship, social security number and date of birth for each of your eligible dependents you wish to enroll in any line of coverage.

You will be able to enter the system when open enrollment begins on November 18th.

Educational Call Center:

Educational Benefit Counselors will be available to assist you with your Benefit Elections for 2025, starting November 18th through December 6th by calling toll-free (855) 937-3332, between the hours of 9AM – 6PM, Monday through Friday.

During your call, the Benefits Counselor will:

  • Provide benefits education and assist you with making your benefits elections
  • Educate you on the supplemental benefits
  • Review your current enrollment information
  • Update all your individual information

You are not required to talk to a Benefits Counselor, but it is recommended that you participate in Open Enrollment.

NEW  BENEFIT GUIDE FOR YOUR REFERENCE

To assist you during open enrollment, we have created A Benefit Guide that provides an overview of your available benefits.  Throughout the Benefit Guide, you will find links to the Benefits Website, carrier portals, how to find a network provider and our annual notices are also included.  This will be a useful resource during the year that you can reference when you have questions regarding the benefits.  The Benefit Guide can be found by clicking on “Benefit Guide” at the top of the tool bar or under the Insurance Documents 2025 section.

BENEFIT UPDATES

  • No change in carriers or plan changes for the Life, Medical plans, Dental and Vision benefits.
  • New Spousal Coordination of Benefits Provision and Surcharge applies for employees hired prior to 8/1/22.  Spouses with available healthcare coverage sponsored by their employer, business organization or any retirement plan, must enroll in such group insurance coverage. If this applies to your spouse, they can be added to your Mentor Medical Plan with coverage as the secondary payer of benefits. If your spouse is eligible for other coverage but you want him/her to remain as primary under the Mentor medical plan, your premium deduction will include an additional $100 per month for a Spousal COB surcharge.  Refer to the Benefits Overview & Costs Tab under the Dependent Rules Section for additional details.
  • Employee contributions for the PPO plan did increase slightly. See below or refer to the Certified Rate Sheet or Classified Rate Sheet for 2025 contributions located under the Insurance Documents section.
  • NEW Supplemental Products being offered effective January 1, 2025
    • Legal Services offered by ARAG
    • Identity Theft offered by LifeLock with Norton

Medical Options

Option 1 Medflex: Available to all insurance eligible employees

  • Medflex is a network only plan with only emergencies covered outside the network
  • Medflex does NOT include the Cleveland Clinic
  • Medflex has $0 deductibles, 0% coinsurance with an out-of-pocket max of $500 for single and $1,000 for family (medical and prescription drug copays apply to out-of-pocket max)

Option 2 SuperMed Plus PPO: Available to all insurance eligible employees

  • Coverage similar to our existing PPO plan but includes deductibles, coinsurance and Network out-of-pocket max of $1,500 for single and $3,000 for family
  • Medical & Prescription Drug Copays are the same as the Medflex plan
  • Network deductible amounts are $250 for single and $500 for family
  • Network coinsurance amounts are 10% with a max of $750 for single and $1,500 for family

Option 3 Bronze plan: Available to Administrators and Certified ONLY

  • $0 contribution cost to employee
  • Highest out-of-pocket costs
  • Network deductible amounts are $6,000 for single and $12,000 for family.
  • Prescriptions are subject to the deductible
  • Network coinsurance amounts are 60% with a max of $850 for single and $1,700 for family
  • Network out-of-pocket max of $6,850 for single and $13,700 for family (medical copays apply to out-of-pocket max)

 Dental

Anthem Life Supplemental Life

During open enrollment, Anthem Life will allow currently enrolled:

  • employees in the Supplemental Employee Life benefit to increase their election by $10,000 without EOI up to the guaranteed issue amount of $100,000 or 3 x salary, whichever is less. 
  • spouses in the Supplement Spouse Life benefit to increase their election by $5,000 up to the $25,000 guarantee issue amount (not to exceed 50% of employee election). 
  • children in the Supplement Child Life benefit to increase $5,000 up to the $10,000 maximum (not to exceed 50% of employee election).

If EOI is required, Anthem Life will send you the EOI form.  The completed form should be signed and faxed to Anthem Medical Evidence Underwriting Department at (614) 880-3529 or sent via email:  LifeDisUW_MEU@anthem.com.  After underwriting review, Anthem will email or mail the results to your home.  If approved, your coverage will go into effect the first of the month following the approval date.

Flexible Spending Account Changes

Effective January 1, 2025 through December 31, 2025, the FSA limit has been increased to $3,300.  In addition, the FSA carryover has been increased to $660.  The monthly admin fee to enroll in this benefit is $2.75.
