Mentor Public Schools provides you a minimum of $45,000 term life and accidental death and dismemberment (AD&D) insurance through Anthem Life at no cost to you.

While Mentor Public Schools offers term life insurance, some employees may want to purchase additional coverage.  Refer to the Supplemental Life benefit summary for details.  Note, if you do not enroll in coverage within 31 days of becoming eligible, you will need to submit Evidence of Insurability if you enroll at a later date.

Your Life/AD&D and Supplemental Life benefits reduce 33% at age 70; 50% at age 75.

During open enrollment, Anthem Life will allow currently enrolled:
  • employees in the Supplemental Employee Life benefit to increase their election by $10,000 without EOI up to the guaranteed issue amount of $100,000 or 3 x salary, whichever is less. 
  • spouses in the Supplement Spouse Life benefit to increase their election by $5,000 up to the $25,000 guarantee issue amount (not to exceed 50% of employee election). 
  • children in the Supplement Child Life benefit to increase $5,000 up to the $10,000 maximum (not to exceed 50% of employee election).

NOTE:  you can add coverage if you experience a life event as long as you are currently enrolled in Employee Optional Life coverage.  Child dependents are covered under Optional Child Life insurance until age 26.
