Transfer Instructions:

  1. Complete the HSA Bank Direct Transfer Form.
  2. Submit the form to Optum Bank via
    • Mail:     Optum Bank; P.O. Box 271629; Salt Lake City, UT  84127
    • Fax: 866-314-9795
  3. Optum Bank does charge a 1 time $20 outbound wire transfer fee for this process. 
    • The $20 fee will be taken directly from the available balance when the transfer is processed
  4. Alternatively, you may call Optum Bank at 866-234-8913, option 0 to request your account be closed.
    • If this method is chosen, Optum Bank will issue a check for the remaining available balance in your HSA Account.
    • Optum Bank does not charge a fee to close out your HSA account.
  5. To avoid a tax penalty, you would need to either spend the funds on a qualified medical expense (as defined in IRS publication 502) by the end of the calendar year or you would have 60 days upon receipt of the check to roll those funds over into another HSA account.
