Consolidated Communications offers three medical plan options so you can select the coverage that best meets your healthcare preferences and needs. Medical plan and prescription drug coverage is provided by BlueCross BlueShield of Texas. Prescription drug coverage is administered by Prime Therapeutics.  All BlueCross options utilize the same PPO network of providers.  The PPO is a traditional copay plan, and the two HDP plans are HSA-compatible high deductible health plans.  A Kaiser HMO option is also available for California employees.

Your Dental Plan is a PPO plan that pays benefits both in-network and out-of-network. Although benefit levels are the same for in-network and out-of-network services, in-network providers agree to charge discounted rates for their services. Out-of-network providers may charge higher fees than in-network providers charge, resulting in higher out-of-pocket expenses for you.  When using non-network providers, you are responsible for charges exceeding what the Plan considers reasonable and customary.  Coverage is provided through Cigna.

Flexible Spending Accounts


You may elect to pay for eligible healthcare or dependent day care with pre-tax deposits to the Flexible Spending Accounts (FSAs). Your income reported to the IRS is reduced by the amount of your FSA contributions, which results in lower overall taxes for you.  You may be reimbursed for expenses incurred during the plan year.  You will forfeit any unclaimed money left in your account after the end of the plan year run-out period.  FSA is administered by WEX.
 
Pre-Tax Parking Benefits


Pre-Tax Parking Benefits allow you to set aside pre-tax dollars to pay for expenses related to work-related parking costs, including parking passes.
  • Parking: you may set aside up to $300 per month to be used for eligible parking expenses
  • Parking benefit elections are submitted monthly via PeopleSoft: Self Service > Payroll and Compensation > Voluntary Deductions
  • You can pay for services using your debit card, or by paying out of pocket and submitting a reimbursement request to WEX
  • If you terminate employment, you may submit claims that were incurred prior to your termination. Any unused funds will be forfeited
