Teladoc
How to register for Teladoc Health

You can use Teladoc Health anywhere you have internet access.

  • Visit www.Teladoc.com and click Set Up Account.
  • Enter your name, date of birth, ZIP code, email address, preferred language and gender and click Continue. The system will identify you based on this information. If you’re unable to be identified, you’ll be directed to Teladoc Health Customer Service.
  • On the next screen, enter the required information and click Set up my account. Your registration is now complete! Then, you can complete your profile by clicking on My Medical History. You can enter your history right after registering or you can come back to finish it later. By finishing it when you register, you’ll be ready to request a consultation any time and you won’t have to fill out your medical history when you’re feeling sick. You can download the Teladoc Health mobile app by visiting the App Store® or Google Play™. If you have any questions, or run into any problems when setting up your account, call Teladoc Health at 1.800.TELADOC (1.800.835.2362)

Employees/dependents enrolled in the Catalina Foothills Banner/Aetna medical plan are eligible to use Teladoc General Medicine.  Anyone enrolled in the PPO Medical plan will have a $0 copay when using this service. If enrolled in the HDHP Medical plan will have a $56 copay when using this service.  If a Teladoc physician orders a prescription, the member is responsible for the cost of the medication.  

Employees/dependents enrolled in the Catalina Foothills Banner/Aetna medical plan are eligible to use Teladoc Primary360 and Behavioral Health.  Members must be 18 years of age to use these services.  

Primary360 Consult Fees:

✓$10 consult fee for employees enrolled in Value Gold, Classic Gold or Copay Gold plans

✓$165/$99 consult fee for employees enrolled in the HDHP “A” Plan per IRS requirement, until deductible is met

Behavioral Health Consult Fees:

✓$10 consult fee for employees enrolled in Value Gold, Classic Gold or Copay Gold plans

✓$90-$215 depending upon type of provider for employees enrolled in the HDHP “A” Plan, per IRS requirement

The PPO Dental plan has a Lifetime Deductible.  The Catalina Foothills PPO Dental plan has a $50 Lifetime deductible.  A Deductible must be paid before benefits are applied to basic and major services.  With a lifetime deductible, you pay the deductible amount only one time, for as long as you are covered by the plan, instead of each year or each visit.  The $50 lifetime deductible is per-person with no family maximum.

program (EAP) at no cost to you.  Up to 5 short term counseling sessions per issue per year. Dependents and partners residing in the employee’s household are covered.
