If you elect an amount above the guaranteed issue, you must complete an Evidence of Insurability (EOI) form if applying for this benefit outside initial eligibility as a new employee. It is your responsibility to complete the form and submit to HR.

The online benefits enrollment should be completed in Munis Self- Service.
This website includes benefits information including Medical Summary of Benefits and Coverage (SBC's), benefit summaries, carrier contact information, forms and more.
Even if you do not accept district benefits, you need to decline the benefits in your Munis Self- Service account.
NOTE: The cost of benefit plans is NOT computed in Munis Self Service. Please refer to the Employee Insurance Rate Chart posted on our web page or under “Resources” in Munis Self-Service
