2024 Summary of Benefits & Coverage (SBC)
1. Complete the following forms sent to you via DocuSign on or prior to your first day:
- Form I-9 - Employment Verification
- W4 Form - Tax Withholding
- New Employee Information Form
2. Complete the Direct Deposit Form (required)
3. Choose your retirement plan and complete the applicable forms:
- Required: DRS Application for Membership as an Elected or Appointed Official (if you are choosing MEBT or Social Security, please mark “Opt out, or Defer”)
- Required: Visit the MEBT website to enroll or decline participation in the MEBT Plan and designate your beneficiary. Click here for instructions.
- Required: Social Security Administration Form SSA 1945
- Optional: Visit the MetLife Enroll Now Website to enroll in the 457 Plan.
- Default: If you do not choose to enroll in DRS or MEBT, you will be defaulted into Social Security, please remember to still complete the required forms above.
4. Log into Workterra and complete your healthcare enrollments within 30 days of your hire date.
5. Submit Electronic Forms Securely use one of the following methods:
- Send an encrypted internal email to RedmondBenefits@redmond.gov
- Send from your personal email via Leap File, click here for instructions
- HR Fax (425) 556-2129
- Drop off in the HR Secure mailbox (located outside of the HR door on the 3rd Floor)
