This PPO (Preferred Provider Organization) plan offers a wide choice of providers.
You can elect to use a provider in the Aetna Choice® POS II (Open Access) network
or any other provider for your health care services. If you choose a network
provider, your out‐of‐pocket cost will be less (please see the plan highlights on page
11 for the difference in coverage between in‐network and out‐of‐network). You do
not need a referral for specialist care. Some services may require preauthorization.

The High Deductible Health Plan (HDHP) has the same Aetna Choice® POS II (Open
Access) as the PPO plan. Deductibles are higher under the HDHP plan and apply for
most services (with the exception of preventive care and certain preventive
medicines). Premium costs, however, are lower. You can find HDHP/HSA providers
online or by phone – please see the information on the back of this Guide under
Contacts Information.

Your Aetna pharmacy plan covers thousands of drugs. To see a list of commonly covered drugs (the formulary), please check the list online. Select the “Drug Search" link below to access the Aetna formulary search tool. “Choose a Plan Year” under the drop down list and select “Premier Plus Plans” then select “View Plan".
If using an out-of-network pharmacy, you will need to pay the drug cost out-of-pocket and then submit a claim form to Aetna to be reimbursed for the amount of coverage.
