Mentor Public Schools provides vision coverage through Ameritas utilizing the EyeMed Access Network to you and your family at no cost for full-time employees.

Effective January 1, 2024, unmarried Child Dependents can stay on the vision plan until age 26.

Go to:ameritas.com to set up your Member Portal to: 

  • Access ID cards 
  • View coverage and benefits 
  • Search providers

 

Register Instructions for First Time Users: 

  • Select blue SIGN IN button in upper right corner.
  • Choose MEMBER SIGN IN under the personal accounts tab. 
  • REGISTER NOW if using for the first time, enter personal information to create your account.
  • Once signed in, select the vision tab to access your membership card and benefits.
