Your Medical Coverage
 
Medical and prescription coverage is an important part of any benefit strategy. Like many employers, our Districts are working diligently to ensure our plans continue to provide affordable, convenient and comprehensive access to quality coverage.
 
Through the Allen County Schools Health Plan, we choose Medical Mutual of Ohio (MMO) for our healthcare and prescription drug benefits.
 
While MMO provides access to participating and non-participating providers, using a participating provider ensures you receive the highest levels of coverage.
 
All plans have important provisions which we encourage you to review. Each plans' details can be evaluated by selecting one of the documents below.
 
Eligible unmarried and married Dependents are covered under the plan until the end of the month of their 26th birthday.
 
Dependent Audit
To provide quality benefits while managing costs, dependent verification is important! Plansource, previously known as Next Generation, is a professional benefit-consulting group specializing in dependent eligibility verification.
 
When adding a dependent to the plan, you will receive a Dependent Eligibility Verification packet from Plansource requesting you to provide documentation such as a copy of marriage certificate or recent federal tax return and birth certificates along with a completed remittance form.

No Surprises Act: This legislation provides rules and processes to protect health plan members from balance billing that may arise from non-network provider utilization that is out of the member’s control. Refer to Surprise Billing Model Notice below.
 
 
Transparency in Coverage Rule: The below link leads to the MMO machine readable files that are made available in response to the federal Transparency in Coverage Rule and includes negotiated service rates and out-of-network allowed amounts between health plans and healthcare providers. The machine-readable files are formatted to allow researchers, regulators, and application developers to more easily access and analyze data.

Dental
Your Dental Coverage
 
At Allen County Schools Health Plan, we partner with Medical Mutual of Ohio (MMO) to provide our employees with access to dental services. Our dental benefit program allows you to use any licensed dental provider. But also offers a network of providers, if selected will provide discounted fees with no balance billing.
 
Most benefits are provided on a reimbursement basis. This means you may be required to pay for services and submit a claim for reimbursement. Claim forms can be found below.
 
It is recommended before having any major dental work done that you have your dentist contact MMO for a pre-treatment estimate. This will ensure that in the event you are having major care performed, you will know the expense and coverage before the service begins.
 
Did you know: you are permitted to elect different types of medical and dental coverage? For example, you can select single medical and family dental if you would prefer.

Allen County Schools Health Plan is providing you and your eligible dependents with an added medical benefit. Teladoc allows you to talk to a doctor anytime, anywhere, by phone or video consult. Allen County Schools Health Plan pays for this service with NO cost to you. It’s a convenient option for treating many of your medical conditions.
 
There’s no need to wait for your Teladoc ID cards you can begin to use the service as long as you are a benefit eligible employee.

The ACSHP Board has contracted with ComPsych® to provide an Employee Assistance Program (EAP) at NO COST TO YOU! Group Life
Your Group Life Insurance Benefit
 
At no expense to you, we provide all eligible employees with life insurance benefits. The amount of your benefit is consistent with your employment contract.
 
This valuable benefit pays a lump sum to any qualified beneficiary(s) you elect. This benefit also pays an additional amount in the event of an accidental death or dismemberment.
 
To qualify for this benefit, you must elect a beneficiary at enrollment. Coverage is underwritten by Dearborn National.
 
Eligible employees may also purchase additional life insurance coverage at a discounted group rate. This additional life insurance can be purchased in $1,000 increments, to a limit of five times your annual earnings or $500,000 (whichever is less). During your intial enrollment, employees can qualify for optional coverage up to $300,000 on a guarantee issue basis. Amounts in excess of the guarantee issue or if you are a late enrollee are subject to satisfaction of the Evidence of Insurability (EOI). The EOI form is available below.
 
If you enroll in the optional life coverage, you may also purchase coverage for a spouse under age 70 or dependent child under age 26. An employee can elect spouse life benefit between $5,000 to $250,000, in increments of $500, not to exceed 100% of the employee benefit amount. An employee can elect child life benefit of $2,500, $5,000, $7,500 or $10,000. Coverage for live birth to 6 months is $1,000 under each of the plan options. The guarantee issue limits are $50,000 for spouse and $10,000 for child life. Please consult the appropriate certificate for your district below under Documents By School for full details.
 
During open enrollment, you may increase your voluntary life benefit up to $10,000 without EOI as long as the benefit does not exceed 5 times your salary or the guarantee issue amount, whichever is less.

Flexible Spending Accounts are an important way to manage your annual expenses while maximizing your take home pay.
 
Flexible Spending Accounts allow you to set aside money to pay for healthcare, dependent care, transit and parking expenses on a pre-tax basis. Money must be allocated to each specific purpose and cannot be reassigned.
