New Hire Benefit Enrollment
Benefit coverage for a new employee is effective the first of the month, following 30 days of employment.
 
To ensure you have coverage, paperwork must be completed and submitted to Human Resources within 30 days from your hire date.

Mid-Year Qualifying Event
Qualifying events include birth or adoption of a child, marriage, divorce, spouse or dependent loss of coverage. If you experience any of these events, you have 30 days from the date of the event to make changes to your benefits. Please contact Sarah Johnson, HR Generalist with any questions or assistance with applicable forms.

You can add or delete eligible spouse or dependents, but you cannot make plan design changes mid-year.

Leave of Absence
Do you need to request a leave of absence from work? Review the process and make sure to complete any forms that may pertain to your need for leave due to personal reasons or a non-work related injury/illness . If you have any questions, contact Megan Martin , HR Generalist or Sarah Johnson , HR Generalist.
 
 
Need to request a leave of absence? Complete the Leave of Absence Request Form and submit it to your Supervisor for initial approval. Once you receive approval, this form should be sent to your HR Representative (see below). Your HR Representative will review your request and provide you with any additional paperwork that needs to be completed.

Work Injury
If you are injured at work, it is important that you notify your supervisor immediately. Paperwork must be completed and submitted to Human Resources. If you have any questions, please contact Megan Martin, HR Generalist.
 
ALL FORMS SHOULD BE HAND DELIVERED OR SCANNED TO HR WITHIN 24 HOURS FROM THE DATE OF INJURY TO ENSURE TIMELY PROCESSING.
 
Employee Forms
 
Employee Report of Injury must be completed and submitted to your supervisor within 24 hours from the date of injury.

Supervisor Forms
 
Supervisor Report of Injury must be completed and returned to Human Resources, along with the employee's Report of Injury form within 24 hours from the date of injury.
 
When treatment is received outside of the recommended clinic(s), the supervisor should fill in the necessary fields on the City of Eagan Letter to Provider . The employee should then provide the letter to the treating provider.
