BCBS Vision Benefit - PPO & HMO Members
Your medical plans include a vision benefit. In order to receive the benefit you must be enrolled in one of the medical plans. If you are enrolled in the PPO plan you may visit any provider, but you must submit a claim form to receive reimbursement. If you are enrolled in the HMO plan, you will receive your vision benefit via EyeMed. You will receive an EyeMed card to share with your EyeMed provider. Please visit eyemedvisioncare.com/bcbsil to find an in-network provider.
