2025 Open Enrollment
Glenview SD 34's 2025 Open Enrollment is from October 28 to November 8. This is the once-a-year opportunity to make changes to your benefits. During this time, you can enroll in benefits for the first time or drop coverage, add or drop dependents, and switch plans. All changes go into effect January 1st, 2025. Your elections cannot be changed until the next open enrollment unless you experience a qualifying life event.

Enrollment Forms
If making any changes to benefits, or enrolling for the first time, choose the form for your position.
To enroll in the FSA, download the form below. Remember, if you are enrolled in single coverage in either HMO plan, in order to receive the $500 district contribution to the FSA, you must complete the form.
To enroll in the Health Savings Account (HSA), download the form below. The HSA is ONLY for employees enrolled in the HDHP 3300 Plan.
