Livongo Condition Management Program
If you're enrolled in the PPO/HDHP, you have access to 2 programs that can help you and/or your covered dependents manage your diabetes or Hypertension, with no out-of-pocket costs. The programs are billed as preventive, allowing the supplies to covered at 100%. Eligible members are outreached directly via mail and/or email. If you have not been outreached, but believe you're eligible, contact the number on the back of your BCBS PPO ID card.
For members with diabetes, you have access to Livongo Diabetes Management program which offers a free connected blood glucose meter and unlimited test strips, at no cost to the member enrolled in the program. Members have access to coaching, alert reminders and summary reporting that can be shared with your provider. See the flyer for more information.

Members with hypertension can enroll in the Livongo Hypertension Management program and receive a connected blood pressure cuff, at no cost. Members will have access to coaches, reporting to be shared with providers, and also receive reminders. To learn more about the program, see the flyer below.
