Health Partners - Dental
 
Avesis - Vision Should you elect this benefit, as a member of Avesis you will have access to many discounts beyond frames and lenses such as Lasik and hearing discounts through Amplifon USA , to name a few! This plan is for materials only because BCBS has you covered for your annual eye exam.
Flexible Spending Accounts - You are eligible to make pre-tax contributions towards a medical and/or dependent care flexible spending account, up to the IRS annual limits. These accounts are administered by WEX
Annual FSA Maximums
  • Dependent Care FSA - $5000 (per household) / $192.31 per 26 pay periods
  • Health FSA - $3200 / $123.08 per 26 pay periods
  • Limited Flex Spending Account - $3200 / $123.08 per 26 pay periods.
*A Limited Flex Spending Account is for an employee whose spouse is contributing to an HSA in 2024, and these funds may only be used for dental and vision expenses.
 
The Hartford - Voluntary Life Insurance
 
  • Every year you have the opportunity to enroll in voluntary life insurance for yourself, your spouse, and/or dependent children.
  • Employee - Increments of $10,000, up to a maximum of $500,000​
  • Spouse - Increments of $5,000, up to a maximum of $250,000​
  • Child - Flat $20,000 (live birth to age 26)​
***If you enroll in voluntary life insurance for the first time or increase your coverage by more than one (1) increment, please complete the EOI and submit to The Hartford by November 10th, 2022.***
 
PERA Voluntary Life Insurance through NCPERS
PERA is a member of the National Conference on Public Employee Retirement Systems (NCPERS) Voluntary Life Insurance Plan. Active PERA defined benefit plan members may be eligible for this group term life insurance plan. The coverage may be continued into retirement with deductions taken from the member’s monthly benefit, but the member must be enrolled as an active member. CLICK HERE to learn more!

Below is an explanation of the required notices:
 
  • Medicaid and the Children’s Health Insurance Program (CHIPRA) Notice: This notice offers information to help employees and their children who are eligible for health coverage, but need assistance in paying their health premiums. The notice gives state contact information for Minnesota.
  • Women’s Health and Cancer Rights Act Notice: The notice outlines coverage required by Federal Law in the event of a mastectomy.
  • HIPAA Special Enrollment Rights: The notice outlines future special enrollment rights if you have a qualified status change for those who waive coverage.
  • HIPAA Notice of Privacy Practices: This notice is to advise you that your employer maintains a HIPAA privacy policy and a notice of your employers privacy practice is available to you at any time.
  • COBRA General Notice This notice advises employees of the their right to Continuation Coverage and should be given to new employees or newly covered employees and their covered members
  • Marketplace Coverage Notice: This notice outlines options about Marketplace insurance. Due to all employees when hired
  • Notification of Possible Federal Public Service Loan Forgiveness Eligibility (PSLF): This notice is to advise Minnesota public employees of possible federal public loan forgiveness.

WEX- New VEBA, FSA + DCAP Vendor
WEX - New VEBA, FSA + DCAP Vendor

*Members have approximately 200 days from the initial debit cad transaction to substantiate a claim. If the claim isn't substantiated, the member's benefit card is suspended. Cards may be unsuspended at the request of the member by contacting participant services.
FSA/DCAP
Flexible Spending Accounts (Medical & Dependent Care), require election annually. 
  • Grace period for 2024 funds ends March 15, 2025​
  • Claims can be filed with a date of service through 3/15/2025. Members have 90 days after the plan year end date to file those claims.
VEBA
WEX is the keeper of your claims. BPAS is the keeper of your funds. To access your true VEBA balances and adjust your investments, log in to your WEX account > select Manage My Account under the Home screen. This will reroute you to your BPAS portal.
 
Set Up Your Account
  • You will not have access to your funds until you set up your online account with WEX
  • If you…
  • did not register and email address with Further in the past OR;
  • you do not have an active FSA or DCAP account with WEX, THEN;
  • you DO NOT have an email on file with WEX. You will need to call into the Participant Services team for assistance with the initial log in – Participant Services Phone: 866.451.3399

Debit Cards
  • Two debit cards per enrolled participant are automatically produced and sent out as enrollment details are loaded into the WEX system
  • Cards will come in a plain non-descript envelope to the address we have on file
  • Need additional cards? You’ll be able to request them through your online account
Participant Services
The Participant Services team is available 6 a.m. to 9 p.m. Central time, Monday through Friday. Access to all participant services options are at: wexinc.com/contact/health.
Investments
As soon as VEBA funds are available within participant accounts, you will be able to start investing your VEBA dollars. Participants will need to log in to their WEX portal, then select the “Manage My VEBA” tab, which will bring you into BPAS’ website through a single sign on. From there, you’ll be able to manage your funds.
