When you enroll in the Aetna medical HMO, PPO or HDHP, you will receive your prescription drug benefits through CVS Caremark. Your prescription drug benefits will be dependent on which medical plan you have elected. CVS and it’s Caremark division are a subsidiary of Aetna, and their preferred Pharmacy Benefit Manager. While you will still be able to pick up your prescription drugs from a variety of in network pharmacies, CVS Caremark will now be the exclusive specialty pharmacy at which you can order and pick up specialty and mail order prescriptions.

Life insurance pays a benefit (called a death benefit, which is usually a lump sum) to a beneficiary (whomever you choose to receive the benefit) after your death. If you have a life insurance policy on a family member (such as your spouse or your children), you would receive the money if that family member died. This money can help replace your income. Your family can use it to pay daily living expenses, the mortgage or rent, funeral expenses, college tuition, etc. And your survivors usually do not have to pay income tax on the life insurance benefit.

Accidental Death and Dismemberment (AD&D) insurance pays you or your family a benefit if you are injured or killed in an accident. It’s not fun to think about, but it is an important benefit that helps to protect your finances in an unfortunate situation.

AD&D typically pays a benefit if you: 
• Die, but only if your death is the result of an accident (not a suicide, for example) 
• Are injured in an accident. The benefit amount depends on how severe your injury is. There are different levels of benefits for losing limbs, losing your vision or hearing, being paralyzed, etc. Your schedule of benefits will show you all of the details.

ELIGIBILITY
You are eligible for coverage if you meet any one of the following:
Class 2- Certified Employees and Administrators actively working a minimum of 10 hours per week.
Class 3-Non-Certified Employees actively working a minimum of 10 hours per week.
Class 4- Retired  Certified Employees at least age 55, but less than age 65.

            *Retiree Benefits do not included AD&D

Temporary or substitute employees are not eligible. 

If eligible, Certified employees - coverage begins the first day of the month following date of hire, Non-certified employees - coverage begins the first of the month following 60 days of employment.

Voluntary life insurance gives you the chance to buy a greater amount of coverage through your company—it’s usually offered in addition to a basic plan that may be company-paid.

It is very important that you choose a beneficiary when you sign up for voluntary life insurance. If you can purchase voluntary coverage for your spouse or children, you will automatically be the beneficiary—but if you are purchasing coverage for yourself, you must name a beneficiary to be sure your benefit goes where you want it to.

Some plans require you to name your spouse as your beneficiary if you are married.

If you have an illness or injury that keeps you from working for a long period of time, you may be able to protect your finances with the help of Long Term Disability (LTD) benefits. LTD benefits provide income replacement when you are unable to work because of an illness or injury. In other words, you still receive a portion of your paycheck, even when you can’t work. LTD benefits usually take over when Short Term Disability benefits end.

ELIGIBILITY
All full-time employees are eligible to enroll in this plan the first of the month following their date of hire.

ELIGIBLE DEPENDENTS
For a Dependent Care FSA, the IRS considers a dependent as any child claimed as a tax dependent on your federal income tax return age 12 and under or over age 13 and disabled.  A spouse or other dependent of any age (such as parent) who is physically or mentally unable to care for himself or herself.

IRS RESTRICTIONS
Because of its tax advantages, rules and limitations are clearly defined by the IRS (including eligible expenses).

  • Use it or lose it - carefully estimate the amount you want to contribute. The IRS requires you to forfeit any amounts not spent by the end of the year.
  • The contribution election you make when you enroll generally must stay in effect for the entire calendar year. You cannot increase, decrease or cancel your contributions during the year unless you have a change in family status (e.g., marriage, divorce, birth or adoption of a child, etc.).

You have the opportunity to save for retirement by participating in your Employer’s 403(b) retirement plan. A 403(b) plan is a retirement plan for certain employees of public schools, tax-exempt organizations and ministers. Contributions are made under a Salary Reduction Agreement (SRA) with your employer. This agreement allows your employer to withhold money from your paycheck to be contributed directly into a 403(b) account for your benefit. Usually, you do not pay income tax on these contributions until you withdraw them from the account.

Spouse Insurance Reimbursement Plan

If a subsidy is available for your spouse's premiums and/or claims:

Premiums are reimbursed once a month.  A signed Premium Reimbursement Affidavit is required in order to be reimbursed.  If there is a change in premium, please complete a Premium Reimbursement Affidavit and return to Marnie Lalor in Human Resources.  The plan will reimburse in-network deductibles, coinsurance and copays based on District 155's in-network benefits.  Claims are reimbursed twice a month.  Submit a copy of the Explanation of Benefits (EOB) from your spouse's insurance plan indicating how the claim was processed.  The EOB, accompanied by a claim form is required for payment. (Claim Form can be found below)

Should there be a change in your spouse's employment, insurance company, medical benefits, and/or contributions, you must notify the District within 31 days.

Want to receive premium and claim reimbursements faster?  Complete the DIRECT DEPOSIT form, attach a voided check and mail it to Group Administrators at the address is on the form.  All premium and claim reimbursements will be deposited directly into your checking account.  No more waiting for reimbursement checks in the mail!
